My Inhealthcare is our smartphone app which enables patients to track, monitor and manage their health remotely.
The convenient app removes the need for patients to attend frequent routine appointments and gives them the freedom to manage their health on the go. My Inhealthcare tracks and monitors patient information such as weight, blood glucose, heart rate, blood pressure and INR to help to manage various long term conditions including COPD, heart failure, obesity, diabetes and undernutrition.
The app includes a medication reminder and scheduled appointment reminder service, allowing patients to keep all important information in one handy place.
My Inhealthcare integrates with existing clinical systems including EMIS Web and SystmOne, meaning readings are visible to the care team. If readings fall outside of parameters alerts are generated to make intervention where necessary.
The app is activated by a secure token which is generated by the GP from within the patient record, meaning registering patients onto services can be done with one click referral.
Patients use a secure and memorable 6 digit login code to access the app, which complies with e-GIF Level 3 information governance standards.
